Copies of relevant documents in support of ownership of works claims are attached. I warrant and represent that all the information furnished on this application is true. I acknowledge that any contract entered into between me and TTCO or by representation contained in this form/ application and any contract so entered will be subjected to cancellation if any question herein contained is not answered fully or accurately. I understand that if my application is accepted as a member of TTCO, (be it under singular performance or annual performance contract), I will be subjected to the By-laws of TTCO and that my rights, obligations and liabilities as a member will be governed by the said By-laws and all rules and regulations made hereunder and accept TTCO as my copyright organization legally representing me and / or my Group/ Band in all matters relevant to the above and in continuity.

Permission Section:

I do give permission by virtue of assignment of my live performance under (Neighboring Rights) to TTCO, use of my photographs for the purpose of TTCO’s website and printed material. TTCO agrees that my rights regarding use of my picture/s without my written approval would represent a violation as far as this section and therefore, only approved pictures may be used accordingly

If you are registering a band, please sign as the band manager and supply all names of the band members currently. Should any member of your band change during the life of this agreement, be advised, it is the duty and obligation of the band manager to inform TTCO of said change/s via email, followed by a phone call to the TTCO’s office, within a 24-48 hours of said change of a member/s

If the application is under 18 years of age, the parent or legal guardian must sign below and on both copies of this agreement.
